An Affidavit of Support and Consent AND a Special Power of Attorney are used when a minor child is applying for a passport, VISA or travel document without a parent or legal guardian. It proves and declares that the affiants, (usually the parents) are giving someone else consent to do something and guaranteeing (financial) support to carry out the action.
Requirements:
- Original of a VALID IDENTIFICATION CARD (preferably passport or driver’s license, etc.) of the child’s parent. The name on the identification must match the name of the signer. Only one form of ID per signer is required.
- Photocopies of a VALID IDENTIFICATION CARD of the child’s parent for each document set.
- Original PSA Birth Certificate of the child/children
- Photocopies of the PSA Birth Certificate of the child/children for each document set.
- Original(s) of the document and all enclosures. Your request will require two forms obtained via this link: Consular Forms – Embassy of the Republic of the Philippines
- Special Power of Attorney: Special-Power-of-Attorney.pdf (multiple children can be included in this document)
- Affidavit of Support and Consent: Affidavit-of-Support-and-Consent.pdf (one for each child)
- Confirm how many documents need to be legalized. If you live outside Jacksonville, it is recommended that you have an extra document legalized in the event that your documents are lost or damaged.
- Applicable processing fee (non-refundable) of $25.00 per document set, payable in cash only. Personal checks and cards are not accepted.

- Please note that witnesses can be obtained in the Philippines.
- Documents that are already Notarized in the US WILL NOT BE PROCESSED at the Consulate. A new form will need to be completed and signed in front of the Honorary Consul.
- Please do not staple your documents together.
- Additional convenience fees will be charged if requirements are not met.
