GENERAL REQUIREMENTS
- Original of VALID IDENTIFICATION CARD (preferably passport or driver’s license, etc.).
- Photocopies of VALID IDENTIFICATION CARDS (preferably passport or driver’s license, etc.). Photocopies of the identification cards will need to be made for each copy of the document you will need to have legalized.
- Original of document (s) (SPA, AFFIDAVIT, etc.) and all enclosures. If an attorney has not provided the documents to you, please see below to download a blank form. PLEASE NOTE THAT THE FORM MUST BE COMPLETED EXCEPT THE PRINCIPAL SIGNATURE (the person signing the document). BLANK FORMS WILL NOT BE ACCEPTED AND YOU WILL NEED TO RESCHEDULE YOUR APPOINTMENT.
- Cash for payment of Applicable processing fee (non-refundable) of $25.00 per gold Consular Seal applied for the document set. Personal checks are not accepted. Refer to FAQ for more information on fees. Additional charges may be added for copies or for convenience fee, if needed.
- NOTE: Witnesses are not required to come to the appointment. They may sign in the Philippines.
LEGALIZATION PROCESS
- Client will create a DOCUMENT SET that contains ALL OF THE BELOW DOCUMENTS:
- Original document (SPA, Affidavit, etc) to be legalized
- Supporting documents (Birth Certificate, Marriage Certificate, Land Title)
- Photocopies of identification for the person or persons to sign the paperwork. All staples should be removed.
- The DOCUMENT SET and Original ID will be presented to the Consular Officer.
- The original documents will be stamped, dated and signed by the Consular officer.
- Client will sign the documents as directed by the Consular Officer
- After signing, a document code will be assigned, and the documents will be scanned and held in secure storage for the Philippine Embassy.
- An Acknowledgement or Jurat Document signed by the Consular Officer will be attached to the front of the group of papers and a Gold Seal will be attached.
- Client will pay for services
- Documents will be received on the same day.
*** Please note that a “RED RIBBON” will NOT be attached to the Acknowledgement as they were discontinued by the Department of Foreign Affairs in 2019 *

General Power of Attorney
The General Power of Attorney (GPA) is a document authorizing another person (called the agent) to perform some service or do something in representation or on behalf of another (called the principal). The service or task to be performed is limited to the administration of the business or affairs of the principal.

In GPA, the agent’s authority covers the administration of all the businesses of the principal. If more specific transactions are needed, a Special Power of Attorney should be used.

The Special Power of Attorney (SPA) Is a legal document granting authority to an appointed person (called the agent or attorney-in-fact) to perform specific acts or transactions on behalf of another (called the principal). It differs from a General Power of Attorney as it is confined to specific matters expressly stated in the document.
Common uses are for real estate transactions where the principal cannot attend personally due to geographical or other reasons or for Overseas Filipino Workers (OFWs) to authorize family members to perform specific transactions on their behalf.

An affidavit is a written statement that a person, called the affiant, makes by choice and swears to as true in front of the Philippine Consul or notary public.
They are used to provide evidence in legal proceedings, help with government transactions, or confirm details in legal documents. They are especially valued because they are sworn to, and so are highly relied upon in an official situation.

An Affidavit of Support and Consent is used when a minor child is applying for a passport, VISA or travel document without a parent or legal guardian. It proves and declares that the affiants, (usually the parents) are giving someone else consent to do something and guaranteeing (financial) support to carry out the action. A Special Power of Attorney must also be used with this document.
**In addition to bringing the original and photocopies of the parent’s identification, the Birth Certificate and photocopies must also be brought with the Affidavit of Support and Consent Document.**

An Affidavit of Support and Guarantee is used when an individual (the “sponsor”) declares that they will provide financial support, accommodation, and other forms of assistance to a traveler (often a relative or dependent) visiting or staying in the Philippines. The affidavit is used to show to immigration or other relevant authorities that the traveler will not become a public burden or otherwise violate immigration regulations while in a country outside of the Philippines.
Affidavit of Undertaking
An Affidavit of Undertaking is used to Submit Requirements, wherein affiant declares that he is willing to comply with all the documentary requirements as specified on any announcement, when formal, sworn commitments are required, such as in business transactions, regulatory compliance, or legal proceedings. An Affidavit of Undertaking typically includes detailed information about the affiant, specific commitments being made, and consequences of non-compliance.
The Affidavit of Identity is a legal document used to formally declare and affirm a person’s
identity, particularly in cases where there are inconsistencies or discrepancies in personal
information across official records.
Common uses include correcting or clarifying variations in the spelling of names, birthdates, or
other personal details found in government-issued IDs, school records, or legal documents. It is
often required by banks, government agencies, or embassies when verifying that documents
bearing slightly different information pertain to the same individual.
